I shall abide by the rules and regulations of MMA. I am aware that my application is subject to acceptance by the Executive Committee of the Association. Should I fail to adhere to the rules and regulations of MMA, disciplinary action can be taken against me. Furthermore, I agree that the above information can be displayed and disclosed to any member of the Association. I will not invoke the Privacy Act against the Association or the Executive Committee Members of the Association. *
